Consent to the collection and processing of personal data

By ticking the corresponding box in a form on ksor.kz, I freely, of my own will and in my own interest, give my consent to the Public Association “Kazakhstan Society of Oncologists and Radiologists” (BIN 250440024031, address: Republic of Kazakhstan, Almaty, 469/2 Seifullin Ave, apt. 200; hereinafter the Operator) to collect and process my personal data in accordance with the Law of the Republic of Kazakhstan No. 94-V of 21 May 2013 “On Personal Data and Their Protection”.
1. Personal data covered
Surname, first name, patronymic; e-mail address; phone number; country and city; place of work, position, specialty; information on education, academic degree and qualifications, including copies of diplomas, certificates and statements that I upload; payment details (amount, date, payment status — without bank card data); technical data (IP address, cookie data, browser details); any other information I provide in the site’s forms.
2. Purposes of processing
Reviewing the application to join the Society and maintaining the register of members; creating and maintaining a personal account; registering for events and issuing certificates; accepting donations and membership fees, accounting; responding to enquiries, including via feedback forms and partnership requests; newsletters about the Society’s events and activities (subject to separate consent; you may unsubscribe at any time); compliance with the legislation of the Republic of Kazakhstan.
3. Processing operations
Collection, recording, organisation, accumulation, storage, updating (revision, modification), use, disclosure in cases provided for by law, anonymisation, blocking, destruction — with or without the use of automated means.
4. Transfer to third parties
The Operator may transfer data to parties that support the operation of the Society’s website and services (hosting, e-mail services, payment organisations) to the extent necessary for the stated purposes, and to state authorities in cases provided for by law. I agree that the use of such services may involve cross-border data transfer in the manner established by the legislation of the Republic of Kazakhstan.
5. Term
This consent remains in force until the purposes of processing are achieved or until it is withdrawn. Data are stored for as long as necessary for the stated purposes and for the periods established by law, after which they are destroyed.
6. Withdrawal of consent
I may withdraw my consent by sending a written request to info@ksor.kz or to the Operator’s address. Withdrawal is not permitted where it contradicts the laws of the Republic of Kazakhstan or where processing is necessary to fulfil obligations, including a contract. Withdrawal does not affect the lawfulness of processing carried out before it is received.
7. My rights
I have the right to know whether the Operator holds my personal data, to receive information about their processing, to demand their correction, completion, blocking or destruction if they are incomplete, outdated or processed unlawfully, and to appeal against the Operator’s actions to the authorised body or a court.
